At LA Community Health Center, your care team will coordinate the care you and your family receive in our centers. We’ll keep your complete medical history on file so we’ll be able to help you quickly and efficiently every time you visit. We’ll remind you when it’s time for check-ups, tests or treatments and help you set goals to manage long-term conditions. If you need referrals, tests or specialists, we’ll help you get the process started.

Our services include Adult Medicine, Family Medicine, Pediatrics, Behavioral Health, Dental, Eye, Radiology, Mammography, Pharmacy and specialty services that include Podiatry, Dermatology, Cardiology and Pulmonology.

Case managers will help assess your health, coordinate care and work closely with home health agencies and community-based agencies to assist you with important services like transportation, housing, utilities, food and managing your daily life.

Our team will provide you and your family with care that is evidence-based and follows scientifically proven guidelines. You will be offered routine screening tests and other health prevention services as recommended by the United States Preventative Health Services Task Force (USPSTF), the foremost group in the U.S. for advising medical professionals in providing preventative care.
